Had this Equinox code Po219A fuel trim balance.  Flushed the injectors  twice, replaced spark plugs ( car had

130 K miles)  It would take driving about 100 miles to turn the light on  Looked at Identifix and most fixes were to replace the fuel injectors(direct injection)  A few responses replaced the O2 sensors.  So I pulled both O2 sensors and to my surprise the front O2 sensor looked as it had been very hot and the protective metal cover was distorted.

Replaced both O2 sensors and the vehicle is fixed.

What do you think caused the heat & distorion. I suspect there may still be a root problem. Lean cylinder conditions, poor fuel spray pattern, or restricted catalytic converter are all possible causes.
